2017-2018 Registration and Facility Fee Forms

Here are the registration forms that are needed to enroll your student (or yourself) in one of our programs.
Please download the forms you need, fill them out and email the completed forms back to us at

  1. Please fill out the Facility Fee Form, which lists the only fees associated with our programs.  We provide free music instruction, sheet music, materials, and the loan of instruments for your student. However, this year we have to charge a small fee to pay rent for the Studio One facilities for our program. Your first student is $75 for the year. Your second student is $50 for the year ($125 total for the family) and your third student is $25 for the year ($150 total for the family).  Any additional students in the family pay no additional fees.
  2. If you are a beginning student, please fill out the Waiting List Form only.
  3. If you are a local orchestral instrument player on woodwinds, brass or strings, aged 7 or older, who have had at least one year of instruction, or permission of the director, please fill out the Renaissance Symphony Orchestra Registration Form.

Paying Facility Fee

If you have already filled out your Registration Form, you can pay your Facility Fee by bringing a check (or cash) to the Saturday or Sunday rehearsal.    Or, you can pay right here using PayPal.   PayPal is considered one of the most secure on-line payment systems.
